The application form may be submitted only on the Chamber´s application form for submission of authorization examination. The application may be obtained at the Office of the Chamber or you can download it from the web site of the Chamber (it is necessary to comply with A3 format of the application and to print it double-sided).

The application shall be submitted not later than 30 days before the date of the examination. The deadline is maintained if the application was addressed to the Chamber filled for the post transport on the closing date or the next working day if the deadline falls on Saturday, Sunday or public holiday. The maintenance of the deadline shall be the date in the postmark.
The applications submitted after the deadline shall be considered as submitted for the next date of the authorization examination.

Attachments of the application are:

a) officially certified photocopy of evidence of formal qualification or the original evidence to make a photocopy at the Office of the Chamber,
b)logbook,
c)portfolio,
d) abstract of criminal record not older than 3 months,
e) proof of payment of the examination fee.

After the expiry of deadline for submission of the applications, the Office shall assemble the Senate of Examiners for the preliminary control. After the preliminary control, the applicants are electronically informed of the outcome by the Office of the Chamber.

The examination has two parts: written test and public discussion.
